Please note that we do not see patients with Medicaid or any Medicaid managed care plan, nor are we in-network for The Empire Plan for New York State employees.

If you have out-of-network benefits with any insurance plan, we will happily treat you as a patient. We will usually be able to send claims directly to your insurance company, but if we cannot we will provide you with a detailed receipt that you can submit for reimbursement. All payments are due at the time of service. For most patients only a co-pay is required. Unfortunately, at this time we cannot accept patients without health insurance. Our annual membership fee is due at the end of your first visit. It is our policy to keep a credit card on file to settle any incidental charges, such as services not covered by your insurance plan.
